This is a classic midge pattern that really should be fished as a nymph and not an emerger. Region's pattern is based upon the first RS2 developed by Rim Chung. The name is short for "Rim's Semblance 2". Rim is a great angler and I have been using this fly since he created it over 30 years ago. I have been amazed on how such a small fly can catch big fish.
